Person-centred care supports people to develop the knowledge, skills and confidence they need to more effectively manage and make informed decisions about their own health and care. It is coordinated and tailored to the needs of the individual, and healthcare professionals work collaboratively with people who use the services. Person-centred care moves away from professionals deciding what is best for a patient or service user, and places the person at the centre, as an expert of their own experience. The person, and their family where appropriate, becomes an equal partner in the planning of their care and support, ensuring it meets their needs, goals, and outcomes.

We provides total patient care that considers the physical, emotional, social, economic, and spiritual needs of the person, his or her response to illness and the effect of the illness on the ability to meet self-care needs.

  • We will assess and respond to referrals as quickly as possible.
  • Where a referral is accepted and serves contracts agreed we will:  
  • Complete a full assessment of care and support needs to inform care and support planning.
  • Develop a fully personalised  cares or support plan bespoke to service user and their family.
  • Provide professional qualified experianced, health care workers to provide the support and care needed.
  • Carry out regular care and support reviews to ensure that needs are met as service users situation changes.
  • Liaison and collaboration with other health and social care professionals to support intergrated and holistic care and support delivery.
  • Providing Ongoing support to service user and their families from a local management team.
